Only 20 states scored high on public health emergency readiness in Trust for America's Health's 2026 annual preparedness report. Thirteen states landed in the bottom tier entirely. That means when the next outbreak, mass-casualty event, or pandemic-level crisis shows up — and with a World Cup drawing millions of visitors to U.S. cities this summer, the timeline isn't abstract — 30 states are flying with half an engine. Working folks don't get evacuated to a private hospital. They get whatever the local public health system has left after two decades of budget cuts and lobbying by insurance carriers who'd rather you buy a policy than fund the infrastructure that keeps you alive.
